We use the expressions, either in negative or positive, to discuss good or bad luck caused by being somewhere at the precise moment something happens.

It can be bad: She just happened to be standing right under the window when the glass broke.  A meter away, or five minutes earlier, and no problem.  But as it was, she was in the wrong place at the wrong time.

It can be good: She just happened to be standing on the corner when she noticed a lottery ticket floating by on the sidewalk.  A few feet away, she wouldn’t have seen it; a few seconds earlier or later, and she would have missed it.  Instead, she won $30,000, just for being in the right place at the right time!
